We coordinate with physicians, hospitals, facilities, patients and caregivers to deliver a clear plan of skilled care at home. Services are based on clinical need, professional orders when required, staffing availability and the patient's payment arrangement.
Rehabilitation
Physical, occupational and speech-language therapy focused on mobility, function, safety and communication.
Respiratory Therapy
Respiratory assessment, oxygen and device education, airway-clearance support and pulmonary teaching when clinically appropriate.
Care Transitions
Prompt follow-up after discharge, medication reconciliation and communication designed to reduce avoidable complications.
Chronic Disease Support
Teaching and monitoring for conditions such as heart failure, COPD, diabetes and hypertension.
24/7 Clinical Access
Active patients receive instructions for reaching an on-call clinician for urgent home-health concerns.
